ELIGIBILITY
For the upcoming 2012-2013 school year the United Methodist Preschool is accepting children between the ages of 2 years by August 15 thru 5 years of age. 

APPLICATION AND REGISTRATION
For the 2012-2013 school year our sessions and schedules will be as follows:

MORNING SCHEDULES:
Monday and Wednesday 8:45-11:45 2 years and 3 year old classes. Children do not need to be toilet trained for this session.

Tuesday, Thursday and Friday 8:45-11:45 2.9 by Sept 1st- 5 yrs

Tuesday, Wednesday, Thursday and Friday 8:45-11:45 4-5 yrs

AFTERNOON SCHEDULES:
Tuesday, Wednesday, Thursday and Friday 12:15-3:15 3-5 yrs.

Tuesday, Thursday, Friday 12:15-3:15 3-5 yrs

Additional Programs:
Lunch Bunch 11:45-12:45 T-F and 11:45-12:15 for afternoon session
Enrichment Art, Science, Literature and Motor- Starting dates will be announced in August 2012. Enrichment sessions are Monday mornings from 9:30-12:30 and Wednesdays from 11:45-2:30.

Parents are encouraged to observe a preschool class prior to applying for admission to the school.  After this initial observation session, parents submit an application form, accompanied by the non-refundable application fee. 

Open registration will begin on January 4, 2012 for our 2012-2013 school year.
First priority admission is given to currently enrolled students.  Second priority is given to siblings of past and present students and children of SRVUMC members and constituents.  Third priority is for all other families.

ENROLLMENT
After an offer of admission is made and prior to enrollment, the following must be submitted:

• Identification and emergency cards
• Preadmission health record completed by the child’s pediatrician
• Financial agreement
• Permission form

Please be advised that State law requires:

• A recent medical examination
• TB test
• Proof of current immunizations (DPT, polio, MMR)
• Parents’ report
• Child Abuse form
• Parent’s Rights forms
